Home Care

Our team of dedicated domiciliary home carers pride themselves in the high standards of delivery. This service extends to most levels of required healthcare from the post op to elderly care . We lay special emphasise on people matching, where companionship, support  and  engagement are paramount. Your carer will also be one that has core knowledge of various illnesses such as Alzheimers, Dementia and so on. having detailed knowledge of these various illnesses goes a long way in offering a stronger health care and support. Your pick of available carers will depend on their knowledge and skills delivering care for these particular ailments.

 

Our carers understand the need for comfort and dignity and do their bit to make life easier for those in need of care. Your care manager is there to monitor standards and review your changing needs on a regular basis. we differ from other service providers in our robust monitoring policy. you and your carer are never alone.

Hospital Care

We provide a robust service in the transition from hospital care to home or specialist care units. Some hospital care requires a hand-over to private specialists who get involved with the in-patient care where there is a situation of long term illness or palliative needs. The specialist treatments and medical management is assessed and a care plan set in motion. with a strong link to the medical professionals we are able to use our qualified specialist registered nurses to carry on with the  treatment process through administering of medicines, physiological rehabilitation or management.

We provide nurses who have current registration with the Nursing and Midwifery Council (U.K.) who work under the NMC Code of Conduct and experienced and suitably trained Care Assistants for placement with the most suitable patient.

 

The gradual transition from hospital to home requires expert care with applicable knowledge of particular illnesses or dissablities. this is where our specialists make their mark. home assessment and routines are carefully looked into. we explore all the options to make your life as bearable as possible. whether you choose to have a live-in carer or a visiting carer we make sure your home works best for you , we make sure your home is managed well with a clean and competent environment to. make your life as pleasant as possible. from house hold chores to home cooking we apply individual approaches to individual needs.

Live-in Care

Our live - In carers are hand - picked to suit the customers exact needs. The matched care providers are individuals who are most able to manage a strong relationship with the customer and their families.  they are dedicated to making life as cosy and warm as possible and take their care responsibilities very seriously. Our matching criteria considers personalities, interests and social compatibility as companionship is most important when it comes to relationships between the cared for and the carer. 

The Initial assessment along with a dedicated care plan includes  a detailed check on home access and mobility needs, meal preparations, proximity to essential services, health and safety and all the elements of home life adjusted to meet the need of the patient. You will be advised on preparing the home for planned care and assisted in making sure equipment necessary home appliances are made available.
 

Our live in carers would require a room large enough to store the essentials. A carer residing in your home means total access all ours of the day and cover is arranged as and when the carer needs to leave the home for personal reasons. this will be fully explained by the care manager before we commence with the service.

Care managers would be on hand to ensure both parties are happy with the care arrangement and would prepare regular reports in compliance with service standards and improvement targets.

Respite Care

A most popular service to partner agencies and health providers. We have stand-by staff ready to give the primary carer a much needed break. Our staff are especially trained in co-working and being able to adapt to the primary carers attributes in terms of high quality care. A full assessment is the first part of the handover to make sure all needs are catered for.

Respite care is designed to give carers a break for a limited period of time. Someone else provides care so the carer can go on holiday, attend to everyday activities or just relax. Sometimes a carer might need emergency respite care if, for example, they get sick or need to go to hospital. If you need emergency respite care, we are ready and available to you.

Respite care is meant to be a positive experience, both for the carer and the person being cared for.

Carers sometimes get physically and emotionally tired. It is important that they can have a break so they can be better carers.

Respite care is also for people who are being cared for, such as children or adults with disability, mental illness, dementia, or older people who are frail. It can benefit them through meeting new people, doing new activities and having fun.

Respite care can be from a few hours to a few weeks. You can organise for regular respite care or you might want to have respite care just now and again.There are different ways to access respite care. You can ask your family and friends for help. The local health authority, designated agency  or if you are with us we will organise the respite calendar having the most suitable carer take over your care.  We would carry out another  assessment first and there would be a full debrief from the current carer so as to enable a clean take over making sure your needs are continuously met.
