Semi Independent Living Services

Whilst we know that being in good care can transform young people's lives and give them good life chance opportunities there is still a big challenge to improve the overall outcomes for care leavers.

1st Healthcare is only successful if we can work and support young people to achieve outcomes. We see our success based on the outcomes we achieve:

  • Providing a stable home where young people feel safe and professional workers who care, support and listen.

  • Sustainable placements by identifying issues before they become major problems; seeking to support and care for young people so they are able to stay in their home and reducing the number of broken placements.

  • Each young person will know how to prepare and cook meals.

  • Each young person will be able to budget and prioritise his or her money.

  • Each young person will be actively engaged in education, training or work.

  • Each young person will be able to manage his or her home environment.

  • Each young person will know how to maintain his or her health.

  • Each young person will feel confident and good about themselves and have a plan for their future.

  • Each young person will feel confident in making their own decisions and resilient.

  • Each young person will feel ready and well-prepared to make the transition to adulthood.

  • Each young person will feel able to take on the challenges of adult life, live independently to make a positive contribution.

  • A support and care service which demonstrates good value - Local Authorities can see outcomes which reflect good value delivered by professionals who care.

Our Home is designed to prepare young people to develop independence skills to allow them to successfully transition to independence. We believe that it is important to support young people in making the transition to independence.

For Age: 16 to 18

We, at 1st Healthcare, offer semi independent living services for young care leavers with low to complex needs. This applies to young people with challenging behaviour; those with diagnosed or suspected ADHD conditions and other emotional or mental issues that have caused situations where accommodation and support are needed. This also includes support for young ex-offenders and those with high-risk situations due to gang affiliation or grooming. Our staff undergo continuous training and skills progression in order to provide the highest quality of support to our service users. Our main approach to support is to initially identify current needs and create a new agreed support plan with measurable targets using our care management system (solariscareware.com) to deliver individualized support based on the assessed needs of our young people. 1st Healthcare plans to ensure safe accommodation and a pathway to a sustainable future and self -independence by supporting the development of skills and inherent ability. We aim to develop our young people by instilling a culture of self -worth and future progression. Education and training remains a major target for our clients.

For Age: 19 to 25

We provide a step - down service where care leavers can continue to receive support and accommodation after legally becoming adults. This is basically a floating support. It is available under the request of the commissioning local authorities who find that these individuals are still in need of support. Accommodation is provided, usually in the form of a house share provision where 5 or more hours of support work are arranged.
